Failure-Mode Analysis Arrives in Airline Systems

Airline Systems now includes failure-mode analysis alongside requirements, trade studies and the fleet model.

How it works

Each failure mode records what fails, its effect, and a corrective action. It is scored with:

  • an ARP4761-style severity class: catastrophic, hazardous, major or minor;
  • occurrence and detection scores from 1 to 10.

A risk priority number is computed from severity, occurrence and detection, so the highest-risk failure modes rise to the top. Each failure mode moves from identified to mitigated to closed, and can be traced to the requirement it affects, keeping safety analysis connected to the rest of the engineering record.
